Remove the verbose #ifndef/#define/#endif pattern for maintaining
header idempotency, and replace it with a simple #pragma once.
This simplifies the headers, and prevents redundant changes when
header files get moved.

"using namespace" in a header file propagates the namespace to
the files including the header file. So it should be avoided.
This removes "using namespace" in header files in test.

Some tests might require to have LIBCAMERA_IPA_FORCE_ISOLATION set
to ensure they can test the IPA running in isolated mode. These tests
are likely to leverage CameraTest. The environment variable should
be set before CameraManager::start() call which happens in CameraTest's
constructor. Hence, plumb the constructor with a flag so that the
LIBCAMERA_IPA_FORCE_ISOLATION can be set before CameraManager::start().

Many tests other than the camera/ tests use a camera. To increase code
sharing, move the base CameraTest class to the test library. The class
becomes a helper that doesn't inherit from Test anymore (to avoid
diamond inheritance issues when more such helpers will exist).
